Study design
We conducted a two-stage study. The first study followed a randomised placebo-controlled trial (RCT) to identify factors that may be associated with weight-related adverse events in overweight and obese people.
Treatment
The second study followed a randomised placebo-controlled trial (RCT) to identify factors that may be associated with obesity and/or overweight in people with type 2 diabetes or overweight.
Outcomes
The primary outcome is a composite measure of cardiovascular events, diabetes, dyslipidaemia, heart failure, or other adverse events. Secondary outcomes are weight loss, insulin resistance, blood pressure, and other measures of lipid profiles. A score of ≥3 points indicates a composite measure of cardiovascular events, diabetes, dyslipidaemia, heart failure, or other events.
We will include participants with ≥1 event per month and ≥1 weight loss per month for the RCT and the placebo-controlled trial. We will include participants with ≥1 event per month for the RCT and the placebo-controlled trial.
Participants
Participants will be recruited from the online recruitment site and will have a BMI >30 kg/m2 (mean age 49 years).
Methods
The study is designed to recruit participants from 1 July 2018. The study is a double-blind RCT. The participants will be randomised to receive either diet pills or placebo (both containing orlistat) for 12 weeks in a randomised, double-blind fashion. Participants will then be randomised to a combination of diet pills (either diethline, the combination of diethline and placebo), exercise pills (either diethline, the combination of diethline and placebo), or placebo. The study is not designed to involve weight-loss trials. Participants will not have access to a study card.

If you are pregnant or breastfeeding, inform your doctor before taking Orlistat. Also, if you are breastfeeding, tell your doctor before taking Orlistat. Because of the possibility of experiencing allergic reactions, avoid taking Orlistat if you are allergic to Orlistat. Also, inform your doctor if you are taking other medicines such as warfarin (Lumavik), phenytoin (Dilantin), cyclophosphamide (inoculation in chemotherapy), metformin (inoculation in chemotherapy), vincristine (inoculation in chemotherapy), etoposide (inoculation in immune-compromising situations), and fusidic acid (inoculation in chemotherapy).
